The first step towards a beautiful, healthy smile is to schedule an appointment.  Our office hours are Monday through Friday from 8 am - 5 pm.   We also offer early morning (7 am) and late afternoon (5 pm - 6:30 pm) appointments on a limited basis.

All new patients should contact our patient coordinator, Donna Baptista-Bean at 877-407-2299, extension 226, or complete the appointment request form below. Donna will be happy to help you schedule an appointment!  All existing patients should call us at 617-739-8200.

Hammond Pond Dental Associates accepts direct insurance benefits from Blue Cross/Blue Shield of Massachusetts, Delta Dental and Guardian Insurance.  For all other insurance carriers, we will require payment in full at the time of service.  We will then submit a claim form to your carrier for reimbursement to you.  Payment plans will be offered through CapitalOne Healthcare Finance.

Please do not use this form to cancel or change an existing appointment.  Please call us if you need to cancel.  We require at least 36 hours advance notification to cancel an appointment.  Failure to provide this notification will result in a charge of $50 per hour of scheduled appointment time.
